Fill in a survey and win a Leeds Rhinos VIP Hospitality Package

Fans still have chance to win a VIP hospitality package for four people at a future Leeds Rhinos game by simply filling in a urgent care review survey.

Last month, the NHS in Leeds teamed up with Leeds Rugby Foundation and Leeds Rhinos to urge local fans to complete a new survey and have their say about their experiences of urgent or emergency care services so that they can help improve and shape services for the future. 

By filling in the survey you will be entered into a prize draw for a chance to win a VIP hospitality package for four people at a future Leeds Rhinos game at Headingley Carnegie Stadium.

Urgent care is when you feel ill or have been injured but the condition is not life threatening but still needs to be seen quite quickly. Emergency care is when you have a serious life-threatening illness or injury that needs to be treated immediately. 

Anybody who lives in Leeds or the surrounding areas that has used urgent or emergency care health services in the past twelve months can take part in the survey and help the local NHS plan the way these services are provided in the future.
